To complete the MAA Consent for Medical Treatment and Concussion form you will need to download the document to your computer.
Open the document you downloaded and complete the fillable areas on the form. When the document is completed and ready to save - choose the PRINT option and select SAVE AS PDF. Name the document your child's first and last name, grade, and then MAA Consent. (i.e. John Smith 7th MAA Consent form).
Return to the website where you downloaded the document and on the right side you will see the option to upload the document. Click on upload document and find the saved document on your computer and upload.

Any softball players wishing to play MAA Softball from our neighboring communities/school districts will only be allowed to do so if it makes sense numbers-wise. We invite you to sign up, but please understand that your status will be on hold until we know what we have for numbers at each age level. Thank you for understanding!
ADDITIONALLY, if your daughter is a 3rd grader born in 2009, a 5th grader born in 2007, or a 7th grader born in 2005 and you do NOT feel they’re ready to move up to the next level, they are able to stay down with the other half of their grade. If this is something you think would benefit your daughter more than moving up, or if you feel she just isn’t ready, please contact Kari Kallio Guth ASAP at firstname.lastname@example.org.
If you are encountering any issues with registering your child online, please know that our intent of the registration deadline is not to prevent your son/daughter from playing MAA basketball this year. We understand that the online registration is a relatively new process and we will help anyone that is having problems with registration. We will also make sure that your son/daughter gets to play MAA basketball.
Change isn't always something that is easy, but the online registration is going to help the MAA be more efficient and organized with all of the player's/team's information. We are pleased at how smoothly this change has gone and are appreciative of your patience and willingness to change with us!
The mission of the Mondovi Athletic Association is to produce quality youth through the promotion of skill building in athletics, fitness, sportsmanship and the encouragement of high academic achievement. MAA goals are to encourage and assist in developing in our youth physically, emotionally, and socially throughout their involvement in learning fundamental skills in athletic programs, without regard to athletic ability or financial capability.
